If you are unable to bring your child to their appointment, but have a family member that will be bringing them, please complete a minor authorization form giving permission to that person to bring them. This is also helpful, as we can then discuss the appointment with them. We also have the Updated Medical History form for you to compete for you child’s appointment, so the doctor can have the most updated medical information.
